Ask your friend, what his small 3 has about him that can be exposed? Chat history, sex vid or what?
She knows where he stays, his work place or what?
Anyway, no matter what, I assumed your friend is married thus has more at stake, he should assume the small 3 can cause more damages.
He needs to think how to manage her and gradually let her grows out of him, ie no longer like him to want to break up with him instead. He can start by doing something that she dislikes, eg praising all ladies appearance or figures.
The last thing he should do is to ask for a break up now. She will become ballistic.

But if he go praise other women..do things that his small 3 dislikes.. wouldn't it trigger the attack from small 3?

There's no right or wrong, only for themselves to decide. In case your friend has made some promises or has led her to think there's a future for them.
Meals and hotels are not part of the deal lah. Even if he has been showering her with gifts. All these are not the same like paying a fee that both agree as a commercial transaction.
